The Licensed Practical Nurse (LPN) participates in the implementation and evaluation of patient care for stable and predictable patients of all ages in the home care setting.

Demonstrates knowledge of nursing principles for the patient in the relevant setting and possesses the ability to perform testing to assess data reflective of the patient's status and to interpret appropriately the information needed to identify each patient's requirements relative to the age-specific needs, and to provide the care needed. Demonstrates competency and practices within the full scope of nursing expertise/knowledge and utilizes appropriate age and population specific standards as designated in the home care setting. Ensures the health, comfort and safety of patients. Knowledge of the principles and skills needed for nursing to provide patient care and treatment. Knowledge of medications and their effects on patients. Knowledge of patient education principles to develop a self-care program. Knowledge of common safety hazards and precautions to establish a safe environment, specific to the home setting. Skill in administering a variety of treatments and medications as directed.




Follows safety measures to ensure proper use of equipment and supplies.;Establishes and maintains productive working relationships within and outside of own area.;Contribute to the evaluation of the plan of care by gathering, observing, recording and communication patient responses to nursing interventions.;Communicate relevant information with patients and health team members to ensure quality and continuity of care.;Applies home healthcare operating principles and practices within own position.;Adheres to legal regulations and organizational policies when discussing patient health information.;Responsible for promoting a safe environment for patients, families, staff, residents, and visitors and working collaboratively with the multi-disciplinary health care team to provide the best outcomes for patients.;Promote a safe and therapeutic environment by providing appropriate monitoring and surveillance of the care environment.;Participate in health teaching developed by a licensed practitioner or registered nurse.;Participate and be part of a healthcare team to conduct focused nursing assessments and contribute to the plan of care.;Observe patients, document and report changes in patient condition, such as adverse reactions to medication or treatment, and take any necessary action.;Implement interventions that are assigned, ordered or prescribed by licensed practitioner or registered nurse.;


This is a position in the hospice program where the LPN will be expected to work at the top of their scope of practice. They need time management skills, self-starting skills, ability to work independently or with a team. They need to be patient focused and flexible/adaptable. We will ask this person to help in fill in staffing needs in the hospice and home health program either in the home setting or a facility. Accountability is a must with the ability to stay on task and be accountable to the team with completing work timely.


Graduate from an accredited practical nursing program as a Licensed Practical Nurse.

If position requires IV therapy/management skills, the following is required in North Dakota: graduation from a Board approved AASPN program where IV therapy is part of the curriculum or completion of an IV Therapy course approved by the North Dakota Board of Nursing.

One year of professional nursing experience preferred.

Currently holds an unencumbered Licensed Practical Nurse (LPN) license with State Nursing board and/or possess multi-state licensure privileges. Obtains and subsequently maintains required department specific competencies and certifications. Valid driver’s license and proof of insurance required. Basic Life Support (BLS) for health care providers required.
