Job Summary

Displays ability to apply the knowledge and skills of advanced social work practice, responding to suspected abuse, neglect or violence in accordance with the National Association of Social Work Code of Ethics policies and procedures alongside the appropriate state laws. Possesses an expert level of critical thinking and in-depth psychosocial assessments. Exhibits ability to apply complex concepts to situations. If carrying the correct licensure, may diagnose patients/clients/residents. May provide specialized counseling and support.

Serves as a member of the interdisciplinary team in providing assistance with social, emotional and economic concerns of the patients/clients/residents and families/caregivers, thus enabling them to achieve or maintain an optimal level of functioning. Utilizes appropriate age-related patient/client/resident care protocols relating to the physical and psychological needs of neonatal, pediatric, adolescent, adult or geriatric patient/client/resident. May work closely with the additional clinical staff to achieve fiscally sound and timely continued planning outcomes. Possesses advanced knowledge of the principles of teamwork. Understands the impact of illness on the patient/client/resident and families/caregivers. Demonstrates knowledge and clinical experience in the psychosocial and economic management of problems commonly occurring with an illness and may provide brief therapy as appropriate. Possesses ability to work with growth and development needs of patient/client/resident populations.

Qualifications

Bachelor's degree in Social Work from an accredited curriculum required. If hired prior to January 1, 2009, a bachelor's degree in Social Work or another field is acceptable. If working in Minnesota, other education accepted according to and based upon Minnesota statute. Healthcare and/or mental health hospital experience preferred. Must possess a license in good standing in state of practice. In Minnesota:

  • Licensed Social Worker (LSW) or other allowed credential based on Minnesota Statute

Master’s degree in Social Work from an accredited curriculum preferred. The Medicare Hospice Conditions of Participation require that the social worker either:

  • Has a Master of Social Work (MSW) from a school of social work accredited by the Council on Social WorkEducation; or

  • Has a baccalaureate degree in social work from an institution accredited by the Council on Social WorkEducation; or

  • Has a baccalaureate degree in psychology, sociology, or other field related to social work and is supervised byan MSW as described in paragraph (b)(3)(i)(A) of this section; and has 1 year of social work experience in a healthcare setting; or has a baccalaureate degree from a school of social work accredited by the Council on Social WorkEducation, is employed by the hospice before December 2, 2008, and is not required to be supervisedby an MSW.
