JOB SUMMARY

The Registered Nurse is responsible for utilizing the nursing process (assessment, diagnosis, outcomes/planning, implementation & evaluation) to provide individualized nursing care to designated area. The RN collaborates with other inter-professional colleagues, including physicians, to plan, implement and evaluate care. Documents patient history, symptoms, medications and care provided in compliance with applicable standards/regulations. The RN is expected to utilize technology to enhance quality patient care, to collaborate with physicians and inter-professional team members, effectively delegate to and supervise unlicensed personnel, assume overall nursing accountability for a patient assignment and serve in leadership roles on department/organizational tasks forces and committees. The RN demonstrates competency and practices within the full scope of nursing expertise/knowledge and utilizes appropriate age and population specific standards as designated in the specific care area or unit. Must have effective communication skills to collaborate, negotiate and resolve conflicts, situations, and/or issues with peers, inter-professional colleagues, patients and families. Must have strong clinical reasoning and critical thinking skills to analyze data and appropriately respond to changing situations. The RN functions within the scopes and standards of nursing practice as outlined in the Nurse Practice Act and Administrative Rules in state of practice and licensure. The Sanford Professional Nursing Practice recognizes the Scope and Standards of Practice (2015) and the Code of Ethics for Nurses with Interpretive Statements (2015) as published by the American Nurses Association as the foundation of nursing care delivery and professional conduct.

POSITION RESPONSIBILITIES

Responsible for assisting the Outpatient AMS Pharmacist in coordinating the outpatient Anticoagulation Management Service (AMS) on behalf of Sanford Clinic including Network facilities. The person will assist the Outpatient AMS Pharmacist in guiding the design, implementation and maintenance of the outpatient AMS and ensure it effectively and fully integrates with the inpatient AMS available at SUSDMC. Works collaboratively with pharmacy staff, nursing staff, medical staff, and other pertinent healthcare providers to assure that all patients under AMS care receive optimal care. In collaboration with the Anticoagulation Program Director, Anticoagulation Medical Director, and Outpatient AMS Pharmacist, strives to ensure the services target quality metric goals are met and exceeded. Provides nursing care by following established standards and practices consistent with Sanford policy, regulating bodies (TJC, CMS, etc.) and professional practice standards as well as state and federal law. Visible and readily accessible to the clinical staff in the various medical staff and other clinical departments.

QUALIFICATIONS

Must have a current license to practice as a Registered Nurse in the state of South Dakota and/or possess multistate licensure privileges as required by the position. Must obtain current license to practice as a Registered Nurse in the stat of Minnesota within 3-months of employment in this position. Completion of an on-line anticoagulation certificate course offered by University of Southern Indiana is required within 1 year of employment in this position. Must be a graduate from an accredited School of Nursing. Bachelors Degree in nursing is required. Case Managers hired or promoted after 4/1/2013 who do not possess a Bachelor's degree in Nursing, must create an education plan with proven continuous action toward achieving a Bachelor's degree in Nursing within 24 months from date into position. Failure to obtain a Bachelor's Degree within the 24 month timeframe will be considered as a withdrawal from current employment status at the Case Manager level. Prior experience in an anticoagulation service is desirable.
