JOB SUMMARY

Responsible for coordinating the outpatient Anticoagulation Management Service (AMS) on behalf of Sanford Clinic including Network facilities. The person will assist in guiding the design, implementation and maintenance of the outpatient AMS and ensure it effectively and fully integrates with the inpatient AMS available at SUSDMC. Works collaboratively with pharmacy staff, nursing staff, medical staff, and other pertinent healthcare providers to assess medication use systems, identify opportunities to improve anticoagulation use safety, and institute practice changes, as necessary. Consistently demonstrates leadership and assumes ownership for the implementation of an efficient and effective outpatient AMS. In collaboration with the Anticoagulation Program Director and Anticoagulation Medical Director, ensures the establishment of quality metrics and follow up monitoring and necessary interventions to ensure enhanced safety and effective outcomes associated with the outpatient AMS. Provides pharmaceutical care by following established standards and practices consistent with Sanford policy, regulating bodies (TJC, CMS, etc.) and professional practice standards as well as state and federal law. Visible and readily accessible to the clinical staff in the various medical staff and other clinical departments. Must understand the role of systematic practices in leading to safe and effective patient care and have the ability to implement these practices in multiple settings. Possesses the ability to cooperate at all levels, both inside and outside of the AMS. Demonstrates the ability to understand and verbalize the dissemination of drug information, to develop procedures and plans and to supervise students and residents. Demonstrates excellent written and verbal communication skills. Has knowledge of and utilizes appropriate age-related patient information relating to the pharmaceutical needs of neonatal, pediatric, adolescent, adult and geriatric patients.

POSITION RESPONSIBILITIES

Follows pharmaceutical guidelines, regulations and standards; adheres to safety procedures. Collaborates with other health care professionals to identify therapeutic treatments and advises providers on safe and effective use of drugs for clinical/therapeutic procedures, including benefits and side effects of alternative treatments. Communicates with patients and providers about therapeutic responses, side effects and associated time frames to optimize drug therapy and improve therapeutic outcomes. Identifies medicinal side effects, drug-drug interactions and food-drug interactions. Follows organizational guidelines and procedures by participating in the collection, processing, documentation, and reporting of adverse events.

QUALIFICATIONS

Must have current license to practice pharmacy in the State of South Dakota. When applicable, must have a current license to practice pharmacy in the state of Minnesota and Iowa within 3-months of employment in this position. Board Certification in Pharmacotherapy or obtaining recognition as a Certified Anticoagulation Care Provider is required within 1 year of employment in this position. Must be a graduate from an accredited School of Pharmacy. Doctor of Pharmacy or Masters Degree in comparable area is required. Successful completion of an ASHP accredited residency is desirable. Prior experience in an anticoagulation service is desirable.
