The Licensed Practical Nurse (LPN) will participate in the implementation and evaluation of patient care, under the supervision of a registered nurse, advanced practice provider, or physician.
The Medical Assistant (MA) functions within the administration pre-defined scope of practice guidelines per state location of practice.
The Care Coordination Assistant (CCA) position assists the Patient Centered Medical Home Team in managing and coordinating patient care. LPN: Ensures the health, comfort and safety of patients. Documents a thorough medical history from patient. Contributes to the assessment of patients and administers medications or treatments as ordered. Provides technical support to healthcare professionals as needed. Counseling patients and family members, under the direction of a registered nurse, on prevention and treatment plans. The LPN demonstrates competency and practices within the full scope of nursing expertise/knowledge and utilizes appropriate age and population specific standards as designated in their assigned clinical setting.
MA: Demonstrates open, friendly, accepting, and supportive behaviors with team members. Demonstrates the ability to follow directions. Guides patients in scheduling appointments and laboratory tests according to established processes and policies. Logs drug/medication data into a computer and maintains computerized records. Perform administrative and certain clinical duties under the direction of a licensed provider or registered nurse. Prepare treatment rooms for patient examinations, keeping the rooms neat and clean. Responsible for promoting a safe environment for patients, families, staff, residents, and visitors and working collaboratively with the multi-disciplinary health care team to provide the best outcomes for patients. Understands medical terminology associated with the contents of patient charts.
CCA: Coordinate provider's patient panels by monitoring patient attribution, perform pre-visit planning, schedule office visits, and managing the appropriate chronic disease registries. Promotes quality initiative and improved patient outcomes. Monitors and tracks patient's referrals for health care services. Participates in the care of patients by collecting subjective and objective data from the patient or caregiver. Communicates collected data to the health care team and follows through on the patient's plan of care under the direction of the provider. Demonstrates computer literacy including using Microsoft Office products and electronic health record (EHR). Participates in care for age related patients in all phases of preventative care and health maintenance. Functions within the administration pre-defined scope of practice guidelines.
Graduate from an accredited practical nursing program as a Licensed Practical Nurse.
If position requires IV therapy/management skills, the following is required in North Dakota: graduation from a Board approved AASPN program where IV therapy is part of the curriculum or completion of an IV Therapy course approved by the North Dakota Board of Nursing.
Currently holds an unencumbered Licensed Practical Nurse (LPN) license with State Nursing board and/or possess multi-state licensure privileges. Obtains and subsequently maintains required department specific competencies and certifications.
Must be a graduate of a recognized Medical Assistant program. Prior experience in a healthcare setting is preferred.
Nationally certified or registered as a Medical Assistant (MA).
Certification (CMA) obtained through a nationally approved certification program for medical assistant, including but not limited to, the American Association of Medical Assistants (AAMA), Certified Clinical Medical Assistant (CCMA) through the Nation Health Career Association (NHCA), Clinical Medical Assistant Certification (CMAC) through the American Medical Certification Association (AMCA), or National Certified Medical Assistant (NCMA) through the National Center for Competency Testing (NCCT), etc. Registration (RMA) obtained through the American Medical Technologists (AMT) also acceptable.
Additional state requirements include:
South Dakota (SD) registration as a MA through the SD Board of Medical and Osteopathic Examiners.
North Dakota (ND) registration as an MAIII through the ND Board of Nursing.
MAs working within Minnesota (MN) and Iowa (IA) do not require state registration.
Basic Life Support (BLS) certification required within six months of employment. Re-certification as required.
Graduate of an accredited school (LPN or MA). One year of clinical experience preferred.
