The Director of Professional Practice and Care Management implements strategies to promote a professional practice environment & provides leadership & oversight to the continuum of care sustaining high quality, cost effective outcomes.
Promotes the Development & Implementation of Evidence Based Practices
-Supports the Shared Governance structure to create opportunities to enhance employee engagement through professional development with shared governance councils
-Develops opportunities to enhance staff achievements & recognition within the organization
-Provides leadership & oversight to care management & the care continuum
-Develops & implements evidence-based, standardized tools to support Care Management and the cross continuum of care inclusive of nursing, social work and the inter-professional team
-Leads Experience initiatives, work groups to fulfill the mission of service excellence & which create a culture of passion & commitment to exemplary resident, family, employee experience. Uses survey data to identify, prioritize, plan & oversee improvement efforts in collaboration with the Enterprise Patient Experience teams
-Coordinates the implementation of High Reliable Organization principles aimed at improving organizational safety culture

-Promotes the development & implementation of highly efficient resident/client flow processes
-Collaborates with the nursing, other clinical & medical staff teams on issues related to service delivery, technology & equipment, capital planning, revenue generation & enhancement, & other related administrative operations matters
-Ensures programs & services are in compliance with corporate policies; as well as, state, federal and local regulations
-Demonstrates the ability to coordinate & lead large scale transformational change at all levels of the organization
-Demonstrates program/project management & data analysis skills in care management including population health and alternative payment models
-Directly supervises assigned employee(s).
-Carries out supervisory responsibilities in accordance with the organization's policies, procedures and applicable laws. -Responsibilities include interviewing, hiring, and training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems.
-Graduation from an accredited nursing program. Bachelor’s degree in Nursing required. Master’s degree in nursing or related area is preferred.
-Minimum 5 years’ experience in LTC with progressive leadership experience & knowledge of nursing & clinical operations, professional practice models & standards.
-Experience with care management or Nursing Shared Governance desirable. Valid, unencumbered nursing license in state of primary residence and/or Multistate Compact license.
