Responsible for assessing, planning, coordinating, teaching, and/or evaluating education and training programs within designated assigned areas.
Responsible to coordinate and integrate evidence-based health promotion programming in group settings to augment care coordination efforts with an emphasis on geriatrics.
Coordinates referrals, enrollments and schedules for appropriate population. Expected to continuously develop, monitor, and evaluate all evidence-based programming for quality improvement opportunities. Coordinates and facilitates community education and events. Role will include: coordination of classes, guidance for lay leaders as a master trainer, assisting and or teaching evidence based practice (EBP) programs, and work with community leaders and care team to build complementary workflows.
Must be able to facilitate group process, utilize adult learning principles, use various teaching/learning methodologies and provide role modeling and leadership. Within the education role, individuals may assist with the administrative duties associated with program operations.
Develops alternative methodologies for education based on adult learning principles. Collaborates with leaders, employees, educational facilities, and other community agencies and other consortium institutions. Responsible for assessing, managing and documenting needs of department related to rules and regulations, compliance risk management safety. Includes education focused on safety and education of outside sources. Understands the competency process of validation. Is self-directed and accountable for ongoing learning and education. Facilitates research. Able to analyze and synthesize data to assess and evaluate educational needs. Demonstrates effective collaborative skills with administrators, other educators, and/or health team members to identify learning needs and the resources necessary to meet customer/client needs. Flexible and rapidly responds to and/or facilitates planned organizational changes through the use of educational processes. Willing to assume and able to demonstrate leadership and management skills. Uses adult learning principles to assess, plan, coordinate and/or provide educational programs that meet the needs of customers/clients appropriate to age specific needs, growth and development or role status. Uses research and health related literature to enhance care practice related to age specific needs, growth and development and/or role status.
Assists in developing primary plans, structures and content for training programs.
Coaches one or several individuals or teams on a specific competency or subject area.
Coaches others in decision-making models, processes, and practices.
Contributes to standard practices for problem-solving approaches, tools, and processes.
Demonstrates expertise in quality assurance tools, techniques, and standards.
Designs critical adult learning programs for the organization that are in line with learning and development strategies.
Develops and monitors programs that integrate learning with practice opportunities.
Develops trust and credibility by acting in a consistent, genuine manner.
Implements established patient safety policies and trains employees on these policies.
Optimizes the use of time and resources to achieve desired results.
Promotes a variety of tools and techniques to measure results against learning objectives.
Bachelor’s degree required. Preferably in a clinical field based on licensure, training and development, organizational development, or healthcare field. If degree is in nursing, graduate from a nationally accredited nursing program preferred, including, but not limited to, Commission on Collegiate Nursing Education (CCNE), Accreditation Commission for Education in Nursing (ACEN), and National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA). If no degree, will consider six years equivalent work experience as a potential alternative.
Two years clinical experience in geriatric settings required.
Currently holds a professional and unencumbered clinical license with the applicable state(s) administered examiners and awarded by a governmental licensing agency. Obtains and subsequently maintains required department specific competencies and certifications based on program training needs of the department and population focus. May be required to possess a valid driver’s license
