Leads in the development of comprehensive patient lifestyle medicine plans. Implements nutrition therapies and exercise interventions designed to help manage and reverse chronic disease, in order to improve patient health and wellbeing.
Determines health, well-being, and nutritional status through a lifestyle medicine consultation and readiness to change assessment. Provides virtual and onsite clinic health and wellness education and coaching according to disease state, medical condition and health risks. Ability to deliver high-quality patient service consistently in a professional manner. Initiates and facilitates member communications to ensure members are aware of their wellness benefits, optimizing wellness program participation.
Assists participants to progress through the development of goals, action plans and relapse-prevention plans in order to meet their lifestyle needs and foster responsibility for self-care. Optimizes health and wellbeing and nutritional status by providing nutritional counseling, cooking classes and education, personal training, and group fitness to patients with high-risk health indicators. Exhibits commitment to maintaining confidential notes on interactions with patients and confidentiality with charting details. Connects patients to community and medical resources. Regularly evaluates the success of the department's lifestyle medicine interventions based on participant satisfaction and impact on health and wellness outcomes. Works with considerable latitude leading the collection, analysis and reporting of programmatic outcomes. Provides insight and suggestions on wellness interventions based on best practice. Provides leadership in the development and oversight of lifestyle medicine patient solutions. Creates lifestyle medicine plans for patients encompassing all dimensions of health and well-being.

Bachelor's degree in dietetics or food and nutrition through an Accreditation Council for Education in Nutrition and Dietetics (ACEND) accredited program.
Prefer at least 900 hours of supervised dietetics practice under the supervision of a registered dietitian or nutrition professional.
Licensed or certified as a dietitian or nutrition professional by the State in which the services are performed and currently holds Registered Dietitian (RD) license within the applicable state(s).
Certified Personal Trainer (CPT) certificate or ability to attain CPT within nine months of hire preferred.
