The Sanford Health Pharmacy Department is seeking a full time pharmacist to provide clinical pharmacy services for Internal Medicine patients as a part of our multidisciplinary team at Sanford Health. This position is a Monday-Friday position embedded in our Internal Medicine clinic department.

Essential Responsibilities include working under a diabetes collaborative agreement to initiate, adjust, monitor and change drug therapy based on information obtained through patient interviews and objective monitoring parameters. Provide medication reconciliation to patients upon request form providers. Under policy, authorize medication refills for Internal Medicine patients. Coordinate transition in care phone calls to post-hospital patients. Provide patient education related to medications prescribed to improve patient compliance. Provide Education to providers and Internal Medicine staff based on department needs.

A successful candidate for this role will have experience and/or advanced training in ambulatory care, and the completion of a residency or specialized experience (PGY-1 or PGY-2 in Ambulatory Care) is preferred. Interest in teaching is a plus and will be available for the candidate, primarily via a preceptor role for our APPE pharmacy students and PGY-1 residents. The successful candidate will also have an integral role in developing our approved PGY-1 ambulatory pharmacy residency program, with potential opportunity to become the RPD of the program.


Must be license to practice in North Dakota and Minnesota and a be graduate of an accredited School of Pharmacy with a Doctor of Pharmacy degree. Experience working in ambulatory care and board certification is preferred.


License must be in good standing by the State Board of Pharmacy in state(s) of practice and/or possess multi-state licensure privileges as required by position. . Graduate of an accredited School of Pharmacy. Must have a thorough knowledge of drugs and their actions; knowledge of state and local regulations.
