Provides integration of health care for clients from pre-admission to post discharge, through coordination and sequencing of the client's care. Integration enhances patient flow and interdisciplinary communication promotes early intervention. Seeks to evaluate outcomes based on an integration among established clinical, financial and utilization data. Feels comfortable functioning in structured and unstructured health care settings described as a geographic and/or situational environment that may not have established policies and procedures. Able to work with growth and development needs of all client populations in a clinical area. Assumes the client advocacy role to identify and resolve issues within the health care delivery system. Facilitates communication links among the various disciplines and agencies/stakeholders, including managed care organization case managers involved in the treatment plan.
Identifies resources best suited to treat patients' conditions and contacts appropriate specialists. Identifies how to maintain an efficient work schedule and the value of time management. Establishes and maintains productive working relationships within and outside of own area. Effectively delegates and supervises based on one’s state practice act and an understanding of the concepts of responsibility, authority, and accountability. Delivers written and oral communication, responds to questions and concerns, and produce specific outcomes and impact. Coordinates and manages care for patients, including those with complex needs. Works with multidisciplinary teams to create an overall patient care plan. Responsible for promoting a safe environment for patients, families, staff and visitors and works collaboratively with the multi-disciplinary health care team to provide the best outcomes for patients. Performs age related patient assessments and nursing interventions. Offers effective feedback to patient and family members regarding health. Informs patient and family about plan status and changes based on progress. Implements healthcare policies and ethical requirements when providing clinical services.
This position is full-time, 80 hours every 2 weeks, day shifts.
Graduate from a nationally accredited nursing program required, including, but not limited to, Commission on Collegiate Nursing Education (CCNE), Accreditation Commission for Education in Nursing (ACEN), and National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA). Bachelor's Degree in nursing required or completion of Bachelor's in Nursing within 18 months of hire. Currently holds an unencumbered RN license with the State Board of Nursing where the practice of nursing is occurring and/or possess multistate licensure if in a Nurse Licensure Compact (NLC) state.
