Analyzes medical records for missing documentation and signatures. Utilizes job specific software in analysis and monitoring functions. Compares the documentation in the medical record against required standards and notes deficiencies for physicians. Recognizes the relation of accurate deficiencies to physician suspension. Contributes to the medical staff timeliness and suspension regulations through accurate notation and monitoring of deficiencies in the medical record. Extensive use of the electronic health record. Applies knowledge of disease processes, anatomy, physiology, medical terminology, Joint Commission (JCAHO), state laws, and other regulatory standards in the analysis of the medical record post-discharge. Knowledge of components and completion requirements for the medical record. Ability to analyze documents for completeness. Excellent understanding of computer information system needed to apply to use. Extensive knowledge of anatomy, physiology, disease processes, and medical terminology. Excellent analytical and critical thinking skills. Must be able to interpret and decipher physician and other care giver handwriting and documentation to ascertain if required documentation is present or whether a deficiency for the appropriate physician must be noted. Knowledge of state, federal, JCAHO, and medical staff regulations as they relate to required components of the medical record. Accurate assignment of physician deficiencies to avoid inappropriate physician suspension for delinquency of medical records. Computer proficiency for use in daily work processes. Excellent organization skills. Work is extremely detailed and requires high accuracy rate to meet regulatory requirements. Good customer service skills required as individual will periodically provide back-up to customer service representative. Works in a team environment. Will work extensively with protected health information and is required to adhere to HIPAA privacy and security regulations and policies related to the same.
Position will analyze hospital medical records to ensure documentation is complete.
Hours for this full time position can be flexed between 7 am - 7 pm after training.
Must maintain certification in RHIT by fulfilling continuing education requirements as required by position. If associate is not certified at hire, the associate must become so within two years of the date of hire. Must have an associate degree in Health Information Management, Medical Record Technology or business related field. Must have successfully written the exam to become certified as a Registered Health Information Technician (RHIT) or be able to acquire certification within two years. Prior medical records experience and an understanding of medical terminology is preferred. Must have the ability to interpret and analyze data/documentation, be able to work well with detail, have good organizational skills, be able to prioritize work, have strong oral and written communication skills and have good problem solving skills. Good working knowledge of medical terminology and medical records practices and procedures is required. Must have good computer skills and knowledge of information systems. Must have excellent organizational and communication skills, ability to effectively plan, and ability to prioritize work to meet expectations.
