JOB SUMMARY

Provides nursing triage services and health advice for patients and managed care members over the phone. Answers patient inquiries over the telephone, assessing the severity of the patient's symptoms and medical needs according to triage algorithms. Guides the patient to the appropriate level of care; schedules patient appointments over the phone with appropriate healthcare providers/facilities/physicians. Documents information provided to patients during triage in health record systems; records patients health conditions for follow-up patient care. Maintains and develops triage protocols; ensures patients' healthcare information is updated on time.

POSITION RESPONSIBILITIES

The Triage/Team RN functions within the scope and standards of nursing practice as outlined in the Minnesota Nurse Practice Act. The Sanford Nursing Practice recognizes the Scope and Standards of Practice (2010) and the Code of Ethics for Nurses with Interpretive Statements (2010) as published by the American Nurses Association as the foundation for nursing care delivery.

The Triage/Team RN is responsible for utilizing the professional nursing process to provide individualized nursing care to patients across the lifespan in an ambulatory setting. The RN cares for patients in all phases of preventative care, health maintenance, diagnosis, treatment, and follow-up as they move in and out of acute care settings.

The Triage/Team RN must have the ability to accurately perform remote patient assessments systematically and comprehensively for patient’s individual needs. Triage nurses encourage collaboration by implementing both provider and nurse prescribed interventions in an effort to reduce complications. The triage RN employs excellent communication and intrapersonal skills.

The Triage/Team RN participates in telephone triage which is an interactive process between the patient and nurse that occurs over the telephone. After identifying immediate needs, the triage RN further defines the level of treatment based upon referrals to the appropriate level of care and the personnel and technical resources available.

The Triage/Team-RN is responsible for the coordination of care, patient screenings, patient education, triage, and various other medical interventions. A sampling of some of the specialized clinical skills that may be performed includes, but is not limited to, age related patient assessment and intervention, IV therapy, assessing/triaging patients either in person or over the phone.

QUALIFICATIONS

Currently licensed with the applicable State Nursing Board and/or possess multistate licensure privileges. Functions within the legal scope of practice. Bachelors Degree in nursing preferred. Minimum of three years of professional nursing experience in a clinical care setting preferred. Knowledge of specialty area is preferred.
