Records and documents clients/patients' health progress information for evaluation; establishes therapeutic plans and modifies them if necessary. Educates clients/patients and their family members about treatment plans/schedules and self-exercises to be continued at home.

QUALIFICATIONS

Licensed Physical Therapist within the state of practice. CPR certification within 6 weeks of employment and re-certification as required. BS, MPT, or DPT in Physical therapy required.
