JOB SUMMARY

Reviews medical documentation from physicians and other healthcare providers; assigns diagnostic and procedure codes for symptoms, diseases, injuries, surgeries and treatments according to official classification systems and standards. Providing accurate and timely ICD-10-CM coding of diagnoses, ICD-10-CM and CPT procedure coding, and assigns DRGS or APCs in accordance with official coding standards, regulatory coding compliance guidelines and company procedures. Reviewing and updating medical record documentation to accurately reflect healthcare coding and substantiate appropriate service reimbursement. Conveying coding guidelines to physicians and other healthcare providers to improve the accuracy of medical record documentation. Working with other departments and organizations to assure availability and quality of information used in statistical reporting for local facility management and helping identify overall healthcare trends, issues and concerns.

POSITION RESPONSIBILITIES

Reviewing and updating medical record documentation to accurately reflect healthcare coding and substantiate appropriate service reimbursement. Conveying coding guidelines to physicians and other healthcare providers to improve the accuracy of medical record documentation. Working with other departments and organizations to assure availability and quality of information used in statistical reporting for local facility management and helping identify overall healthcare trends, issues and concerns. Position will be coding for Pathology.

QUALIFICATIONS

Must maintain certification in RHIA, RHIT, CCS, CCS-P, CPC or COC by fulfilling continuing education requirements. If the associate is not certified at hire, the associate must be become so within one year of the date of hire. Must have successfully written the exam to become certified as a RHIA, RHIT, CCS, CCS-P, CPC, or COC or be able to acquire certification within one year. Completion of a two-year health information technician program with an associate degree of science degree is preferred. Encoder experience is highly preferred. Must have specific knowledge of diagnostic and procedural terminology, ICD-9/ICD-10 diagnosis and procedure, CPT, and HCPC coding schemes. Must have an understanding of drug groups, adverse effects of drugs, interaction between drugs, and treatment dosages. Must have a working knowledge of anatomy, physiology and pathophysiology to understand disease processes, treatment or management of conditions either medically or surgically. Must also have computer skills, the ability to interpret, analyze and abstract data/documentation, have good problem-solving skills, be self-motivated and have good time management and organizational skills. Must be current in coding schemes and have knowledge of prospective payment systems and clinical practices and technology.
