Responsible for the coordination of care, patient assessment, patient education, triage, and various other interventions. Occasionally works in emergent patient care situations requiring critical thinking and prompt assessment skills. Will perform age related patient assessment and intervention as well as scheduling tests and interventions per physician order. Responsible for utilizing the professional nursing process to provide individualized nursing care to adult and geriatric patients in an ambulatory setting. Cares for patients in all phases of specific states including assessment, disease maintenance, treatment, and follow-up as patients move in and out of care settings. Responsible for utilizing the professional nursing process to provide individualized nursing care to adult and geriatric patients in an ambulatory setting. Must have working knowledge of electronic medical records. May perform rounds with or without physician in hospital and on hospital patients and take call with the physician. May place verbal or telephone orders using read back on hospitalized patients. Abides by respective healthcare agencyΓÇÖs policies. Requires excellent time management skills and ability to prioritize/triage the needs of the patients and the practice. Excellent customer service skills are a must. Must be skilled in assessment and triage of patients with various conditions and devices both in person and over the phone.
